GENERAL INFORMATION

Receiving results, repeat scripts and referrals please ensure that you book a follow up appointment to see your GP/doctor to find out the results of a test or procedure. Your GP/doctor will usually not give results out over the phone.

To ensure that every aspect of your health is considered, and that you are getting the best possible health care, your doctor will normally require you to come in for a face-to-face consultation before providing you with a script or a specialist referral.


Photo ID is required in order to collect any patient information from reception or from your doctor. If a third party such as a friend or a family member comes in to collect something on your behalf, please make sure you let our receptionists or your doctor know the name of that person. This is to ensure you have provided consent for your information to be released to that person in line with privacy regulations.

Medical Certificates
Your GP/doctor will require a consultation with you prior to issuing a medical certificate for medical conditions requiring time off work, and is unable to backdate medical certificates.

Management of your Personal Health Information

Your medical record is a confidential document. It is the policy of this practice and your doctor to maintain security of personal health information at all times and to ensure that this information is only available to authorised persons.  Your doctor and the practice abides by the Australian Privacy Principles available at Australian Privacy Principles quick reference | OAIC.

If you would like to have your medical records transferred from another doctor please ask reception or your doctor and this can be organised for you. Please be aware that most medical centres charge a small fee to transfer medical records to cover the cost of printing and postage.

Home visits
The doctors consulting at Alpha Medical conduct home visits for their regular patients who live within 5km of the practice and cannot attend a face-to-face consultation due to ill health. For further information about home visits, please contact reception or your doctor.

After hours
For medical emergencies, dial 000. For after-hours home visits please call 8724 6300. The closest public hospital to the practice is Blacktown Hospital on Blacktown Road, dial 9881 8000.

Accidents and Emergencies
If you have a medical emergency, please notify reception before you leave to attend this practice as your doctor will be able to advise you of the appropriate course of action and to prepare for your arrival.
 

Telephone access to your doctor
Your doctor may be contacted during their regular consultation hours. If your doctor is with a patient, a message will be taken and the reception staff will advise you when it is likely that your doctor will return your call. If your call is concerning a medical emergency, please let the receptionist know and you will be put straight through to your doctor where possible.


Emails
No doctors consulting at Alpha Medical conduct consultations via email, as confidentiality over email cannot be guaranteed. If you need to speak with your doctor, please call reception and schedule an appointment.


Changing Personal Details 
Please inform reception or your doctor of any changes to your name, address, phone number, health care card, and pension or Medicare number. It is also important to update your next of kin and emergency contact details,


Complaints 
If you are unhappy or have a complaint regarding any aspect of your care, please discuss the matter with your GP/doctor or with reception. You may also contact the Health Care Complaints Commission on 1800 043 159.

Interpreter Service All of the doctors consulting at Alpha Medical Centre have access to a telephone interpreter service. This needs to be booked in advance so please advise reception when booking your appointment with your doctor if this service is required
